A las 21:15h entra en el control de enfermería “Pedro” que es un paciente de 49 años ingresado en la unidad de agudos de salud mental por depresión con ideación autolítica.

Comenta que ha pasado todo el día con ideación recurrente de ruina y desesperanza y que esta mañana se ha guardado una cuchilla de afeitar del aseo para hacerse daño pero que ha conseguido controlar la compulsión sin apenas lesionarse y la entrega.

A la exploración presenta varias lesiones superficiales en ambas muñecas, se realiza cura tópica, escucha activa e intervención del psiquiatra de guardia.

Realiza un análisis de lo sucedido, establece qué barreras han fallado y cuáles han sido las causas. Establece un plan de tratamiento del riesgo adecuado al mismo.
