Una úlcera cutánea crónica es una herida que para su cicatrización requiere de un periodo de tiempo superior a 6 semanas, y lo hace a través de un proceso complejo en el que se elimina y reemplaza el tejido dañado. Normalmente lo hace mediante un proceso de cicatrización por segunda intención y el resultado estético es una cicatriz de gran tamaño con un recubrimiento frágil y sensible.

Las úlceras cutáneas crónicas suponen un importante problema de salud con importantes repercusiones para la calidad de vida de pacientes y de sus familiares y cuidadores. También requieren de un esfuerzo muy importante en la movilización de recursos humanos y económicos de los sistemas de salud.

Para el profesional de enfermería suponen un reto debido a la complejidad del diagnóstico y el tratamiento, a la cantidad de productos disponibles en el mercado para el tratamiento de las mismas y debido, también, a la falta de preparación y experiencia en el cuidado de las mismas.

Con este curso se pretende realizar un acercamiento al concepto y principios del tratamiento de las UCC y abordar de forma integral la Atención al Paciente que sufre una úlcera cutánea crónica. El cuidado no debe centrarse exclusivamente en la úlcera, debe abordar todos los aspectos de atención al paciente, la valoración integral del paciente, el control de la causa que produce la úlcera, los cuidados generales del paciente y los cuidados locales de la úlcera. Los cuidados locales se basan en los principios de la “cura en ambiente húmedo”, abordando el tratamiento local de la herida desde un esquema estructurado que nos permite tomar decisiones sobre el tratamiento de la úlcera sin basarnos en prácticas tradicionales, muchas de las cuales resultan inadecuadas e incluso perjudiciales.

En la segunda parte del curso abordaremos los aspectos fundamentales de las úlceras más comunes: las úlceras por presión y las úlceras de la extremidad inferior: úlceras venosas, úlceras arteriales y úlceras del pie diabético. Finalmente, se describen otras úlceras cutáneas menos frecuentes.
